BACKGROUND

Amendment to Ordinances 77-005 and 95-055 to amend the PUD to allow a 1,612 square foot expansion for a medical office.     No variations required.

RECOMMENDATION

A public hearing was held by the Plan Commission on March 23, 2016 where Commissioner Ennes moved and Commissioner Drost seconded:

A motion to recommend to the Village Board of Trustees approval of PC# 16-003, an Amendment to Ordinances 77-005 and 95-005 to amend the PUD to allow a 1,612 square foot expansion for a medical office.This approval is subject to the following conditions:

1. The plat of easement for cross access and drainage shall be recorded prior to issuance of a building permit.

2. Prior to Village Board, the plat of easement shall be modified so that the drainage easement is to the benefit of Lot 1, not the Village.

3. Prior to issuance of a building permit, the detention basin shall be modified to meet the requirements of the MWRD Permit 77-496 and Petitioner shall verify if Structure #25 includes the required restrictor, which shall meet code.

4. An executed onsite utility maintenance agreement shall be provided prior to issuance of a building permit.

5. The Petitioner shall comply with all applicable federal, state and local codes, regulations and policies.

Roll Call Vote:  Commissioners Ennes, Drost, Cherwin, Green, Sigalos, Warskow and Chairman Lorenzini voted in favor. Motion carried.
